On this page you will find the necessary documents for applying for membership in Ukrainian Dental Union (UDU).
To apply, you need to download, print, fill out, and send us the following documents:
- Application form for joining the “Union of Dentists of Ukraine” – contains basic information about the candidate, contact details, and professional information.
- Application for joining the Ukrainian Dental Union – an official document confirming your desire to become a member of the organization.
After filling out and submitting the documents, the application is considered by the UDU Board, and you will be notified of the results additionally.
